And by 144hz I don't actually mean running it at 144fps. I'm well aware that the actual engine and physics are tied to the framerate and all that and can't go beyond 60 and that's fine. Thing is I'm running a 1060 and want a locked framerate that ISN'T 30 at 1440p. 60fps at 1440p though is too much for my card without disabling literally everything, and even then I still get drops at certain key areas. It is "bearable" but I really want a lock at all times. So, what I thought of doing was locking it to 1/3 of my refresh rate, 48fps since the game never drops below that even maxed out at these key areas and it would mean a lock at a still quite smooth and responsive refresh rate. Problem is, the game locks the refresh rate itself to 60hz even when I set it to 144hz on Unsqueezed. So obviously it's probably doing some heavy level driver stuff, and quite frankly it's a good thing that it makes sure the monitor is at 60hz so it's all in sync and is paced properly IF you can get that lock. But again, I can't, and wanna have a properly frame paced and synced 48fps.

See, you are actually missing the point here but it's alright since it's a little bit of a confusing problem so I don't really blame you. But here's the deal: To get a properly frame paced and synced signal to the monitor, you want the game to be running at a consistent frame rate that's consistent with the intervals and timings of your monitor. So, in this case, I have a 144hz screen meaning I can lock something to 48fps and it'll look perfect, no jutters or stutters since it's essentially a perfect third of my refresh rate. And that's what I'm looking for, since it's the highest perfectly synced frame rate that will look good on my monitor and is a framerate that I can actually still reach. That's probably the reason the game locks to 60hz, because it'll only reach that framerate and if you would have any other refresh rate such as 144hz that 60fps wouldn't look proper or be synced up hence you'd get weird incosistent stutters. So, my goal isn't to run it beyond 60fps, my goal is to get the game to still have the screen run at 144hz so that I can lock it below 60fps, that is 48fps in my case. That's why I said "and by 144hz I don't actually mean running it at 144fps". Any game, even if it can't go beyond 60, should still be able to send a signal that the monitor will take as 144hz even if it's still 60. So again, the engine only reaching 60fps or only being able to render 60fps properly isn't the problem and has nothing to do with what I'm bothered by. I just want it to run my monitor at 144hz so I can lock the games framerate to 48fps for the best most consistent experience possible with my hardware.

That's a fair idea and I've been thinking about that stuff, however that would still run the monitor at 60hz in the game whilst having v sync trying to reach 60fps, and if I then set riva tuner or something to 48fps that means it'll be constantly thinking it can't hit 60fps, turn off v sync and have horrible tearing all over the place and still be running my monitor at 60hz meaning it'll be the same as running 60fps at 144hz - it won't be synced up with my monitor and it'll look stuttery as all hell. At least to my eyes, most people probably can't notice that stuff but I'm way too sensitive and spoiled ;D.
However, a solution I did find was running it at borderless window. This means it can't mess with my refresh rate and will just use what's currently being sent out by windows and my nvidia driver. And then through that Unsqueezed program I can lock it to 48fps, and using triple buffering vsync and what not it actually looks pretty much perfect. The game has such good motion blur and with it being perfectly synced up with my monitors refresh rate it really still kinda looks like 60fps on a 60hz screen except the FPS never drops and it still looks amazing maxed out and 1440p. So, I think I've found the best of both worlds though if there is a way to do the same thing in fullscreen I suppose that might be good to know since I'm not entirely sure if there's any incompatibilities between v-sync and borderless window. Ah well, as far as I can tell it's basically perfect in every way so I guess this might be considered solved :).

I haven't encountered anything like that, ladders work, gravity works, lifts work, special weapon arts work.
I think enviromental things work just like normal but when the patch tries to sync your movement speed according to the framerate, occasional bugs can happen, I think that's the only issue here.
EDIT: Damage over time spells like pestilent mist and heal over time items, rings and spells also seem work as intended. Also status effect build up works normally, farron keep swamp poison damage tick works normally as well.
A user by the name of SaucyX has released a patch / mod to DS3 that allows you to run the game at 144fps and beyond!

Способ был найден игроками, и, вроде как, помогает повысить производительность, правда, он не спасает от “падения” клиента игры в определенных моментах. Мною способ был опробован, но на 780Ti разницы нет никакой, так частота в Dark Souls 3 зафиксирована на 60FPS, и даже на максимальных настройках производительность на высшем уровне 99% игрового процесса.
Но вы можете опробовать этот способ: Перейдите в “Панель упраления nVidia”, выберите параметры 3D приложений и добавьте туда Dark Souls 3.exe, чтобы иметь возможность настройки.
Затем прокрутите список до самого низа и найдите параметр “Пре-рендер кадров Виртуальной Реальности” (Virtual Reality pre-rendered frames) и восстановите значение “По умолчанию”, то есть “1”
Это проблема не повсеместна, но имеет место быть, так как GPU, почему-то, используется для обработки VR по умолчанию.
